About the CHD Clinic

The CHD clinic covers most aspects of prevention and management of cardiovascular disease, including Hypertension, Pre-diabetes, Diabetes, Stroke, and Chronic Kidney disease.

The clinic is run by Specialist Nurse Practitioner Manager, Elaine James and the lead doctor is Dr Smith.

We run a Coronary Heart Disease (CHD) for patients with or at risk of cardiovascular disease, for example those patients who have suffered a heart attack or experience angina, patients that have had a stroke, and also patients that have a higher risk of developing Cardiovascular Disease, such as those with Diabetes with hypertension (high blood pressure).

Suitability

The completion of this form should only be made if you have received an invite to the digital weight management service and you are eligible for a referral. This includes the following: – Your body Mass Index is greater than 30 (27.5 if ethnic minority) – You are on the diabetes regsiter or have a high blood pressure *
